| Details: Job SummaryThis position is for our Oakmont, PA (Willows & Westminster Place) locations.The LPN position Coordinates and participates in all aspects of resident care while promoting independence, to ensure quality of life. Supervises provides leadership and direction to all Personal Care Aides. Responsibilities Provides appropriate care for residents through assisting with all ADL’s and medication/treatment regime while promoting independence Initiates contact with physician, emergency service and family in event of change in physical/mental status in order to provide essential care Transcribes, executes, coordinates physician order to ensure appropriate care Coordinates resident appointment to expedite care. Assists with developing implementing and evaluating all areas of residents care and needs to ensure quality of care Assists with resident admissions, discharges and transfers for smooth transition Develops, monitors and maintains updated Service Plans Participates in monthly interdisciplinary meetings to enhance resident’s quality of life Responsible for maintaining accurate and updated documentation and records to ensure good communication and thereby provide for effective continuity of care Instructs and supervises Personal Care Aides in Westminster Place policies and procedures to ensure proper technique Supervises, evaluates and assists in counseling Personal Care Aides in work performance in order to maintain quality care Prepares schedules and updates assignments for staff to ensure adequate care Promotes safety in care and surroundings to prevent injury to staff or residents Communicates with the Director of Resident Services on a daily basis to ensure continuity of communication Promotes planned and spontaneous activities with residents by personal care aides Has knowledge of and implements emergency measures to ensure resident safety and comfort in the event of accident, fire or disaster Participates in annual training as required by DPW regulation | ||||
